A clear damage report lets a supplier investigate the actual delivery. Start with the order and affected item, preserve useful evidence and follow the reporting process supplied with the purchase.
Record what you observed
Identify the order, delivery date, product reference and carton label. Photograph the packaging and visible issue, including a wider view that connects the damage to the carton. Avoid including private documents or resident possessions unnecessarily.
Describe the condition rather than deciding who caused it. A damaged carton, damaged product and missing component are different observations. Keep them separate so the supplier can assess the right issue.
Follow the receiving instructions
Use the carrier and supplier’s actual process for noting exceptions and submitting reports. Ask what packaging or components must be retained. Do not discard evidence before understanding those requirements or promise a particular claim result to the owner or resident.
If the product may be unsafe or cannot be used as intended, follow the property’s control process and obtain appropriate assessment. A cosmetic photograph is not enough to decide every safety question.
Make the requested resolution clear
State what is needed to restore the approved setup: assessment, replacement part, repair proposal or another supplier response. Ask about timing and any return or collection process. Keep a potential alternative separate until it is approved.

Update the completion ledger and inventory when the issue is resolved. A claim acknowledgment is not a completed replacement. Record the actual item or part received and any new instructions.
The report’s job is to preserve facts and support resolution. It does not establish liability or guarantee a refund, which depends on the actual circumstances and applicable terms.
